The NEL Personalised Care Programme Team is delighted to invite you to our first NEL-wide Care Coordinator Workshop event on Wednesday 13th July 2022 from 1.30pm – 3.30pm.We recognise the vital importance and role played by Care Coordinators in primary care, to support the needs of our local populations.Our early work has identified some great examples where Care Coordinators are having a real impact. We also know there is more work to do around role definition and further embedding the role in primary care. There are now 50 Care Coordinators in post across North East London, so the time is right to come together to shape the future of this role in NEL.This workshop will give you the opportunity:· To strengthen clarity about what the Care Coordinator role is, its purpose and focus· Share what is working and hear from some examples of great practice· To shape and influence NEL Care Coordinator programme for 2022-23· To look at how Care Coordinators can contribute to health inequality priorities· To collectively explore challenges and how these may be addressed· To look at what PCNs and primary care need to help Care Coordinators thriveAnd of course, this will also be an opportunity to connect and engage with other Care Coordinators & managers across NEL.
