As you may be aware, we have recently published a toolkit to support PCNs and managers to embed the personalised care ARRs roles and use them to tackle inequality.
It aims to:
- Practically support PCNs to recruit, employ, embed, support, and develop all three of the roles
- Signpost to all the useful resources, specifications and guidance on personalised care, social prescribers, health coaches, care coordinators and how population health and health inequalities relates to personalised care
- Give practical examples of how each of the roles can work with patients, different cohorts, including those impacted by health inequalities
We have also published one-pager documents summarising each of the three roles, their unique contribution in tackling health inequalities, and training & support needs:
These tools complement the NHSEI Workforce Development Frameworks for each of the roles, of which the social prescribing version has been published. The Care Coordinator and Health and Wellbeing coach WDFs are due to be published soon. Once these are published, we will share summary version, to help highlight the key elements and direct to information.
