MEETING THEME
An opportunity for our members to get to know each other, share ideas and learn from each other. The meeting will begin with a presentation about Well Newham and then we will have presentations from one person representing each personalised care role before moving into discussion groups.

1:00pm – Katy Szita and Augusta Adu, Co-chairs welcome and share survey
1:05pm – Ysabella Hawkings, LBN Public Health Principal to present about Well Newham as a tool to support residents and how it can support personalised care roles
1:20pm – Social Prescriber presentation
1:30pm – Health & Wellbeing Coach presentation
1:40pm – Care Coordinator presentation
1:50pm – Community Neighbourhood Link Worker presentation
2:00pm – Community Connector presentation
2:10pm – Care Navigator presentation
2:20 – 2:40pm – Break into groups to discuss key questions
Reflective practice, peer support, team meetings and supervision
What can the roles learn from each other re reflective practice, supervision, peer support and should/could we make more space for peer support as a Community of Practice?
What are the barriers we and residents/patients face to resolving their support needs and is there a way our roles can work more collaboratively to address this?
Discussion and agree actions to survey feedback.
