Personalised care roles encompass various professionals who facilitate social prescribing and similar interventions. These individuals dedicate their time to understanding individuals’ needs, focusing on their priorities, and adopting a holistic approach to enhance their health and wellbeing. They link individuals with community groups and essential services to provide both practical and emotional support.
