A PCN enhanced level practice nursing role will work with a range of stakeholders to collaborate at scale for the development and delivery of services to a local Primary Care network, with a specific focus on Population health management.
The post holder will undertake a clinical supervisory role, overseeing a team of immunisers delivering a service within a primary care network to general practice teams care homes and for patients that are housebound.
Key Requirements of Role
• Qualified Registered Nurse from a degree in nursing or from a registered nurse degree apprenticeship (RNDA).
• Registered with the Nursing & Midwifery Council.
• A post graduate qualification at level 7 or above relevant to their area of enhanced practice, public health, and population health management.
• Working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework; and
• Works across the PCN, in services that enhance the core general practice offer to deliver the key responsibilities outlined.
A non-medical prescribing qualification would be beneficial to this role.

Duties and responsibilities
Support the Clinical Directors in enabling nursing workforce development, maximising immunisation and vaccination uptake, workforce capacity, and service improvement across a Primary Care Network.
a) works as part of an MDT within the PCN.
b) consults with patients, their family, and the multi-professional team to undertake assessments of patient need, devise, and evaluate complex care plans.
c) evaluates and analyses clinical problems using their clinical knowledge, seeking out and applying relevant evidence, enhanced techniques, interventions, and equipment to make clinical decisions.
d) delivers enhanced clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies using analysis and their underpinning knowledge to manage complex interventions.
e) teaches and advises patients and their families on how to manage their condition or support the multi-disciplinary team to do so.
f) participates in clinical audits and research projects and implements changes as required, including the development, and updating of practice protocols / guidelines and procedures locally.
g) works within national and local protocols where these exist.
h) recognises boundaries of their practice and know when and to whom patients should be referred.
i) using the principles of delegation, delegates work to other members of the MDT and take responsibility for the delegated activity as appropriate.
j) demonstrates initiative and are creative in finding solutions to problems.
k) holds responsibility for team performance and service delivery; and
l) provides supervision to trainee nurse associate, nurse associates and general
practice nurses.
Clinical Leadership
• Supervise clinical assessments and consent agreements and consult on the provision of enhanced practice advice on vaccines and immunisations across a primary care network.
• Address any concerns that may arise regarding vaccine hesitancy and contra-indications in partnership with a wide range of stakeholders.
• Supervise the review of patients’ medical history and address any potential adverse reaction risks; provide patients with information on what to do in the event of adverse reactions.
• Supervise vaccination delivery (oversee paperwork and vaccine confirmation activity, ensure national guidelines are respected regarding PPE, clinical and non-clinical waste, sanitisation).
• Assess and sign-off competence and capability training of new vaccinators.
• Coordinate the infection control and waste disposal, ensuring that all activities are adhered to the highest H&S standards.
